Eating more grains may reduce liver cancer risk

Dietary patterns are closely related to the risk of cancer, and diet has always been considered an important risk factor for hepatocellular carcinoma (HCC). However, so far only excessive drinking and aflatoxin-contaminated foods have been identified as the main causes of liver cancer. Researchers have turned their attention to whole grains and dietary fiber.

Whole grains are the main source of dietary fiber and consist of the bran, germ and endosperm, while refined grains only contain the endosperm. Whole grain foods include whole wheat bread, whole wheat crackers, oatmeal and cooked grains such as quinoa, brown rice and millet.

Increased intake of whole grains and dietary fiber may reduce the risk of obesity, type 2 diabetes, and nonalcoholic fatty liver disease, all of which are known risk factors for HCC. In the United States, higher intakes of cereal fiber and bran may be associated with a lower risk of HCC in adults, according to a large observational study published online February 21, 2019 in JAMA Oncology.

Research Methods

The cohort study of 125,455 participants included data from two large prospective studies - the Nurses' Health Study (NHS) and the Health Professionals Follow-up Study (HpFS), including 77,241 women and 48,214 men.

Participants filled out food questionnaires every two years, covering food categories including whole grains (bran and germ) and high-fiber foods (cereals, fruits and vegetables), and consumption frequency ranged from never, less than once a month, to six times a day or more.

Study Results

After a mean follow-up of 24.2 years, a total of 141 patients were diagnosed with HCC (70 women and 71 men). Increasing whole grain intake was significantly associated with a lower risk of HCC (hazard ratio [HR], 0.63; 95% CI, 0.41-0.96; p = .04). Eating whole grain bran was associated with a 30% lower risk of HCC, while eating whole grain germ was associated with an 11% lower risk of HCC, but this was not statistically significant. Eating high amounts of fruit or plant fiber did not reduce the risk of HCC.

Conclusions

Conclusions: Higher intake of cereal fiber and bran may be associated with a reduced risk of HCC in US adults. Increasing the intake of whole grains or cereal fiber may be a primary prevention strategy for HCC in the future. Future studies also need to consider the impact of hepatitis B and C virus infection and this association in other racial/ethnic or high-risk groups.
