1. Symptoms of urinary obstruction

Large tumors, tumors at the bladder neck, and blood clots can cause urinary incontinence or even urinary retention. Tumor infiltration of the ureteral orifice can cause upper urinary tract obstruction, resulting in low back pain, hydronephrosis, and renal impairment.

2. Late-stage tumor manifestations

When the advanced tumor invades the tissues and organs around the bladder or has pelvic lymph node metastasis, it may cause bladder pain, urethro-vaginal fistula (vaginal fistula: an abnormal channel formed between the reproductive organs and the urinary system, manifested as urine leakage), lower limb edema and other corresponding symptoms. When the tumor metastasizes to distant places, it may also cause impaired function of the metastatic organ, bone pain and cachexia.

3. When the tumor is large, the mass can be palpated by vaginal or rectal bimanual palpation

However, this method is not accurate enough, and bimanual palpation may not be able to examine all parts of the bladder. It is even more difficult to examine the poorly relaxed abdominal wall clearly. In recent years, with the advancement of imaging, this examination has been rarely used.

4. Hematuria

Painless macroscopic hematuria is the most common symptom, which may occur in more than 80% of patients, of which 17% have severe hematuria, but 15% may only have microscopic hematuria at the beginning. Hematuria is mostly intermittent throughout the whole process, and may also manifest as initial hematuria or terminal hematuria. Some patients may excrete blood clots or necrotic tissue. The duration of hematuria and the amount of bleeding are related to the malignancy, stage, size, number, range, and morphology of the tumor, but they are not necessarily proportional. Carcinoma in situ often manifests as microscopic hematuria, while hematuria in urachal carcinoma of the bladder may not be obvious. Bladder tumors of non-urothelial origin may not have hematuria if the lesions do not penetrate the bladder mucosa.

5. Bladder irritation symptoms

Frequent urination, urgency, and pain during urination account for about 10% and are related to widespread carcinoma in situ and invasive bladder cancer, especially when the lesion is located in the trigone of the bladder. Therefore, patients with long-term "cystitis" that cannot be cured should be alert to the possibility of bladder cancer, especially carcinoma in situ.
