What is primary brain lymphoma? What are the treatments?

Primary brain lymphoma is rare, and the following are relatively special in its imaging manifestations: it is more common in middle-aged and elderly people, and the incidence in men is higher than in women; the lesions are mostly solitary but can also be multiple; plain scan CT often shows equal or slightly high density, and MR often shows equal or slightly low signal on T1-weighted scanning and low signal on T2-weighted scanning, with equal, low or slightly high signal; enhanced scanning CT and MR both show single or multiple mass-like uniform enhancement; the lesion outline is clear, with umbilical notches visible at the edge; mild edema is seen around the lesion, and the range of edema is inconsistent with the size of the lesion; the space-occupying effect of the lesion is mostly mild; the lesions are mostly located around the ventricles, often close to the midline.

What are the treatments?

1. Chemotherapy has a certain effect on some PCNSL patients with normal immune function. However, how to determine which patients are effective for chemotherapy, the choice of chemotherapy regimen, the timing and dose of intrathecal injection, the dose of radiotherapy, and the neurotoxic side effects of drugs all need further research.

2. For immunocompromised patients, especially AIDS patients, the primary disease is the main treatment. When most AIDS patients are diagnosed with PCNSL, their general condition is extremely poor, their immune function is low, their sensitivity to radiotherapy is poor, and the dose of radiotherapy cannot be too large. All these factors make the efficacy of radiotherapy for AIDS PCNSL patients significantly lower than that for patients with normal immune function.
