What causes blood in stool?

1. Diseases of the digestive tract itself, such as ruptured and bleeding esophageal/gastric varicose veins, stomach and intestinal ulcers and inflammation, parasitic infections, tumors (including polyps and cancer), hemorrhoids, intussusception, anal fissures, dry stool abrasions, etc.

2. Diseases of other systems besides the digestive tract, such as blood diseases, acute infectious diseases, vitamin deficiency, poisoning or drug toxicity, etc.

3. Children with bloody stools are mostly caused by intestinal polyps. The blood is bright red, painless, and the blood and stool are not mixed. It can also be seen in bacterial dysentery, intussusception, blood system diseases, etc. Bloody stools in adults are mostly caused by hemorrhoids, anal fistulas, anal fissures, intestinal polyps, intestinal cancer, inflammatory bowel disease, etc.

Clinical manifestations

1. Bloody stool

Most of the time it is acute (immediate) bleeding, and the blood flows out of the blood vessels and is discharged through the anus with feces in a short time, or flows out directly after defecation. The appearance of the blood flowing out is similar to that of bleeding from external injuries, and its color is bright red, purple red, or dark red. It can coagulate into blood clots after a while. Common in the following diseases:

(1) Hemorrhoids of all stages, both internal and external hemorrhoids and mixed hemorrhoids can cause bleeding in the stool, usually with fresh blood in the stool or blood dripping after defecation. External hemorrhoids generally do not cause bleeding in the stool.

(2) Intestinal polyps cause painless stool bleeding. Bleeding occurs during bowel movements and stops after bowel movements. The amount of blood varies. Generally, the blood does not mix with the feces. However, if the polyps are located high and the number is large, they may be mixed with the feces.

(3) Long-term rectal prolapse may cause bleeding during defecation.

(4) Anal fissure with bloody stool. The bleeding pattern is blood on one side of the stool surface that is not mixed with the stool. Some patients have blood dripping after defecation.

2. Bloody stool with pus/mucus

That is, the excreted feces contains both pus (mucus) and blood. Purulent (mucous) and bloody stools are often seen in tumors and inflammation in the rectum or colon. The following diseases are common:

(1) Rectal cancer causes fresh or dark red blood, and there may be mucus in the stool. Blood, mucus, and stool are often mixed together.

(2) As the course of colon cancer progresses, bleeding in the stool will gradually occur. Most of the time, the stool is bloody and contains pus or mucus, and the blood is dark in color.

(3) Ulcerative colitis causes mucus or bloody stools, accompanied by left lower abdominal pain or lower abdominal pain.

(4) Intestinal infectious diseases such as bacterial dysentery, amoebiasis, etc.
