In fact, the treatment effect of strabismus in adults is not very good, so generally speaking, strabismus should be discovered and treated as early as possible. When treating strabismus, it is important to consider surgical treatment, especially for adults. 1. Strabismus examination method 1. Routinely check visual acuity, corrected visual acuity, eye position, eye movement, fundus and visual function. 2. Visual acuity was checked using a standard logarithmic visual acuity chart. Those with a visual acuity below 0.8 underwent mydriasis refraction examination using a ciliary muscle paralytic. A total of 75 cases of refractive error and 42 cases of amblyopia were detected, including 13 cases of hyperopic refractive error, 52 cases of myopic refractive error, and 10 cases of mixed astigmatism. 3. Use corneal reflection method and prism plus alternating cover method to check the 33cm and 5m oblique angles respectively. In the esotropia group, patients with an esotropia angle of 50° underwent additional shortening of the main external rectus muscle. The first choice for the exotropia group is monocular bilateral lateral rectus muscle weakening surgery, and for those with >50△, the main strabismus medial rectus muscle shortening surgery is added. All operations were performed under local anesthesia. During the operation, the eye position was observed to be correct or slightly undercorrected, with no diplopia as appropriate. 2. Treatment of strabismus The diagnosis and treatment of strabismus in adults is the same as that in children, and most require surgical treatment. The surgery requires finesse and precision, but it is not a major operation and is not expensive. For acquired paralytic strabismus, treatment should be directed at the cause. If conservative treatment fails after half a year and the condition is stable, surgical treatment may be considered. Because adults are cooperative during preoperative examinations, local anesthesia can be used for surgery, and the clinical cure rate of the surgery is very high. However, because their treatment is too late, their functional cure rate is lower than that of children. 3. Postoperative Care 1. Both eyes will be bandaged after the operation. The patient should be advised to close his eyes and rest, and move his eyeballs as little as possible to avoid affecting healing. 2. The bandage can be opened 1 to 2 days after the stitches are removed. 3. Use a sterile saline cotton swab to clean the eye secretions and scabs every day, then use 0.25% chloramphenicol eye drops, apply 0.5% chloramphenicol eye ointment on the incision and conjunctival sac, and finally apply a fixed bandage. 4. If the pain is severe, you can take painkillers orally; if you feel nausea and vomiting, you can take chlorpromazine orally. 5. After strabismus correction surgery in adults, you should eat a light diet and avoid strenuous exercise. |
