Can perianal abscess be treated without surgery?

Many patients are confused about whether perianal abscess should be treated with surgery. In fact, if the perianal abscess is serious, surgical drainage is recommended. If surgery is not required, the patient should also follow the doctor's advice on conservative treatment.

Surgery and drainage treatment is recommended

1. It can prevent the infection from spreading throughout the body and endangering life.

2. It can prevent the infection from spreading to the surrounding area, causing horseshoe-shaped abscesses and then forming complex anal fistulas.

3. It can relieve pain.

4. It can prevent damage to the anal sphincter and affect anal function. The purpose of surgical treatment for perianal abscess is to completely incise the abscess cavity, allow the pus to flow out, and cause the abscess cavity wall to fall off, and then fill the abscess cavity through granulation growth, and finally cause the abscess to subside.

5. Treatment of simple abscess: In the lithotomy position or side-lying position, use local anesthesia or spinal anesthesia to make radial incisions at the abscess site. After releasing the pus, insert the index finger to explore the size of the abscess cavity and separate its intervals. If necessary, cut the skin at the edge of the incision a little to facilitate drainage, and finally insert a Vaseline gauze strip into the abscess cavity for drainage.

6. Abscess in which the abscess cavity is connected with anal fistula: After opening the abscess, carefully check the internal opening with a probe, then open the fistula tract, appropriately remove the skin and subcutaneous tissue, and slightly remove the tissue around the internal opening to ensure smooth drainage. If the internal opening is deep and the fistula passes through the anal sphincter, thread hanging therapy can be used. The advantage of the above surgery is that the abscess is cured in the first stage and no anal fistula will be formed. However, in acute inflammation, when it is difficult to find the internal opening, you should not search blindly to prevent the spread of inflammation or the formation of a false channel. Only make an incision and drain the pus, and then perform anal fistula surgery after anal fistula is formed. ?
