The drugs used include glycerin/sodium chloride (compound glycerin) lotion (glycerin, 95% ethanol, distilled water), antipruritic tincture (liquefied phenol, mint, salicylic acid, 75% ethanol added to 100ml), calamine lotion (calamine, zinc oxide, glycerin, calcium hydroxide solution 100ml), etc.

4. Use antihistamines, commonly used ones include diphenhydramine and chlorpheniramine.

Differential Diagnosis

This disease must be differentiated from pruritic papules and plaques of pregnancy. The characteristics of the latter are: more common in primigravida, onset after 36 weeks of pregnancy, severe itching. Skin lesions often occur in stretch marks, with small red papules 1 to 2 mm in size, which soon merge into larger red plaques. It is more common in the abdomen, and then urticaria and papular rashes spread to the buttocks, thighs, and chest, and the face is rarely affected. The disease in pregnant women does not affect the fetus, and the rash usually disappears on its own a few days after delivery. Because the pathological changes in skin tissue are similar to those of prurigo gestationis, they can only be distinguished by the difference in clinical manifestations. The diagnosis can be made based on the time of onset, distribution of the rash and characteristics of the lesions. However, it should be differentiated from erythema multiforme, insect bites and herpes gestationis.

1. Erythema multiforme

It is a self-limiting skin disease with a polymorphic rash that is characterized by a target-shaped or iris-shaped rash that can invade the mucous membranes and is easy to distinguish.

2. Insect bites

It is often a wheal-like papule with papules or small blisters in the center of the rash, and there is a clear history of arthropod or insect bites.

3. Herpes during pregnancy

It is an autoimmune disease that can occur at any stage of pregnancy. The rash is erythematous, papulovesicular or dermatitis herpetiformis-like rash, mostly distributed on the palms, trunk and limbs, but it does not tend to occur in stretch marks. Direct immunofluorescence examination of the maternal lesion and surrounding skin showed linear C3 and IgG deposits in the basement membrane, which can be distinguished from PUPPP (pruritic urticarial papular plaques of pregnancy).

Itching during pregnancy is a unique symptom during pregnancy. Although it may include certain skin diseases with a good prognosis, it needs to be carefully identified to rule out skin diseases caused by viral infection that seriously threaten the lives of pregnant women and fetuses (embryos) and have strong teratogenicity, such as rubella, herpes gestationis, and impetigo herpetiformis. Pregnant women suffering from the above-mentioned skin diseases must terminate their pregnancy and should not blindly seek treatment to preserve the fetus, so as to avoid serious consequences.
