What to do if double eyelid surgery fails

In life, many women are particularly fond of beauty and pursue perfection. They are already quite good-looking, so they must follow the trend and do some beauty projects. The most common one is double eyelid surgery. Although the technology for double eyelid surgery is now mature, due to various reasons, some women still fail to achieve the ideal effect after the double eyelid surgery.

1. Fatigue in opening eyes.

Before the operation, the patient may have mild ptosis or weak levator palpebrae superioris muscle; during the operation, the incision may be too deep and damage the levator palpebrae superioris muscle aponeurosis; early scar hyperplasia and contracture may cause it. Ptosis, difficulty in opening the eyes and a feeling of heaviness, excessive surgical trauma, damage to the levator palpebrae superioris muscle, hematoma, bleeding, infection causing tissue adhesion and scarring.

Correction method

Different correction methods are used for different reasons. For those with levator palpebrae superioris muscle injury, the muscle is shortened. For those with scar contraction, scar softening and adhesion loosening are performed. For those with heaviness in the eyes without any eye manifestations, psychological treatment is performed.

2. Incision scar.

The reasons are incision infection, uneven and poorly aligned incision edges, or the patient has a scar-prone constitution.

No obvious double eyelid line

It manifests as partial or complete disappearance of the double eyelid line, which is more common in the buried suture method and suture pad method. The so-called machine double eyelid pressure and film pressure method on the market are actually a type of buried suture method. The reason for failure is incorrect method selection. For those with thick and swollen eyelids, the buried suture method fails to achieve permanent adhesion of the tarsal plate and skin at the double eyelid line. The orbital septum fat and orbicularis oculi muscle before the tarsal plate were not completely removed, and the skin and tarsal plate failed to adhere.

Correction method

For those whose double eyelids have completely disappeared, they should have the surgery re-done according to the method that suits them. For those with thin eyelids and no or light excess skin at the inner corners of the eyes, the buried suture method can be chosen. For those with thick eyelids (blister eyes) and a lot of excess skin at the inner corners of the eyes, the minimally invasive method or incision method should be used instead. For those whose double eyelid lines have partially disappeared, only the disappeared part can be re-operated. Of course, the curvature should be kept consistent and natural with the remaining double eyelid line.

3. The appearance is not beautiful.

The patient has a short palpebral fissure, resulting in wide double eyelids and round eyes; the orbital resection at the lower edge of the incision is not enough, resulting in bloated and unnatural double eyelids; the shape of the double eyelids is not in harmony with the facial features.

Triangle Eyes

The double eyelid line is interrupted, with poor continuity, too high in the middle, and too high on the inner side of the double eyelid line. When the eyes are opened, the inner side cannot be lifted together with the eyelid margin.

Correction method

Redesign the double eyelid line, widen the width of the interruption and outer side of the double eyelid line, make an incision along the original double eyelid line, remove a part of the upper eyelid skin in the middle and outer side of the upper edge of the incision, so that the width of the inner and outer sides is coordinated.

4. The double eyelids are too wide , too narrow, too long, too short, and the bilateral sides are asymmetrical. The reasons are improper design. The design line is too wide, too narrow, too long, too short, and the bilateral sides are asymmetric. The incision is not made along the design line. The patient's bilateral asymmetry is not taken into consideration during the design.

Double eyelids are too narrow

It refers to the absolute width of the double eyelids being too narrow (less than 5 mm), resulting in hidden double eyelids. Compared with the patient's face shape and palpebral fissure width, the double eyelid width is too narrow and the double eyelids are not coordinated. This is related to the design and suturing. The designer (including the patient's prior request) designed it to be too narrow, or the suture position was too low during suturing.

Correction method

Make an incision along the original incision, fully peel off the skin below the double eyelid line, and peel as far as possible to the gray line at the lower edge of the upper eyelid, remove the orbicularis oculi muscle and pre-tarsal tissue in front of the tarsal plate as much as possible, and suture to make full use of the elasticity of the upper eyelid skin. Lift the incision and suture to increase the width of the double eyelid (the width can be increased by 1 to 3 mm). For young people, this method can appropriately make the eyelashes curl up and increase the beauty. In addition, it strengthens the thinning of the subcutaneous tissue under the double eyelid line to make the double eyelids more natural and realistic after surgery.

5. The formation of triple eyelids.

The reason is that the original double eyelid adhesion line was not separated, and the original double eyelid line and the newly designed double eyelid line formed a triple eyelid, which is more common in the buried suture method; too much orbicularis oculi muscle was removed above the incision, causing the upper eyelid skin to adhere to the deep tissue to form a triple eyelid.

6. Multiple wrinkles on the upper eyelid skin, commonly known as triple eyelids

After double eyelid surgery, two double eyelid lines will appear (one to two more wrinkles than double eyelids), and triple eyelids may appear completely or partially (inside or outside). The cause is that the double eyelid line is not designed according to the arc, the skin between the double eyelid line and the eyelid margin is unevenly peeled off during surgery, the tissue is removed unevenly, too much skin is removed from the inner corner of the eye, too much subcutaneous tissue is removed above the incision line, resulting in multiple adhesions between the skin and the tarsal plate, and the sutures are misplaced and not sutured according to the arc.

Correction method

Mild early triple eyelids may disappear after the swelling subsides; you can also manually pull the skin above the incision for 2 to 3 months to relax it and cover the excess double eyelid line; in severe cases, the original incision should be made, the wrinkles should be peeled off and loosened, and the skin below the incision should be sutured upwards over the adhesion of the original tarsal plate to the skin.
