There are many causes of coccygeal pain. In clinical practice, we are used to dividing it into two categories:

1. True coccydynia: The pain comes from the coccyx itself, for example, the pain caused by a coccyx fracture due to a fall.

2. Pseudo coccydynia: The pain comes from outside the coccyx, such as pelvic inflammation, tumors and diseases of the urogenital system.

Anatomy of the coccyx: The coccyx is usually composed of three to four small bones, and its proximal end is connected to the sacrum by fibrocartilage. Usually the range of motion of this joint is greater in women than in men, especially during pregnancy. The coccyx itself has the coccygeal ligament, gluteus magnus, coccygeal muscles, anal sphincter and levator ani muscles attached to it. In men, the coccyx is located higher than the ischial tuberosity, providing better protection when hit. On the contrary, in women, the coccyx is relatively easy to be bruised when falling because the pelvis is wider and flatter.

Cause of pain: The sensory nerves in the coccyx area come from the fourth and fifth sacral vertebrae and the coccygeal nerve itself. When there is a lesion in the reproductive and urinary system in the pelvic cavity, the pain will involve the coccygeal nerve through the sacral nerve, which is the cause of pseudo-coccygealgia.

Clinical symptoms: In clinical practice, most patients suffering from coccyx pain are young women. Some suffer from acute pain, while others suffer from chronic diseases that have not improved despite long-term treatments. Patients are often unable to sit or stand in a normal manner and always feel more comfortable lying in bed or standing. This pain is sometimes only concentrated at the tip of the coccyx, with local tenderness, but for some people the pain range is more vague. The pain is sometimes exacerbated by increased pressure on the levator ani muscles. If the pain worsens at night or when you cough or sneeze continuously, you should be especially careful, because sometimes tumors near the coccyx or spinal tumors can cause similar symptoms.
