The operator continues to inject hyaluronic acid into the tissue gap outside the blood vessels without noticing. The bleeding and the injection of hyaluronic acid increase the pressure between the tissues. When the external pressure reaches a balance with the systolic pressure of the blood vessels, but exceeds the diastolic pressure of the blood vessels, the blood will flow back, and the mixture of blood and hyaluronic acid in the tissue gap will be pressed back into the blood vessels, forming an embolism.
