The clinical name of twitching of the corners of the mouth is hemifacial spasm, hemifacial spasm, and primary hemifacial spasm. It is neither a precursor to cerebral thrombosis nor facial neurosis.

The clinical manifestation of hemifacial tics is paroxysmal irregular involuntary twitching or spasm of the facial muscles on one side. It usually occurs on one side and may also be secondary to facial nerve paralysis. The cause of primary hemifacial twitch is unknown, but it may be caused by pathological stimulation somewhere on the facial nerve conduction pathway. A few cases are sequelae of facial nerve paralysis.

Symptoms of hemifacial spasm

1. Hemifacial spasm can be divided into two types: one is primary hemifacial spasm, and the other is hemifacial spasm caused by the sequelae of facial paralysis. The two types can be distinguished based on their symptoms. Primary facial spasm can also occur in a static state, and the spasm will be relieved after a few minutes and is uncontrollable; the facial spasm caused by sequelae of facial paralysis only occurs during actions such as blinking and raising eyebrows.

2. Most of the time, it is a paroxysmal involuntary twitching of the orbicularis oculi muscle on one side, which gradually and slowly spreads to other facial muscles on one side of the face. The twitching of the muscles at the corner of the mouth is the easiest to notice. In severe cases, it may even affect the platysma muscle on the same side, but the frontalis muscle is less affected.

3. The severity of convulsions varies, and they are paroxysmal, rapid, and irregular. The convulsions are mild at first, lasting only a few seconds, and then gradually lengthen to several minutes or longer, while the intervals between convulsions gradually shorten and the convulsions become more frequent and severe.

4. A convulsion can last from a few seconds to more than ten minutes, with intervals of varying lengths. The patient feels upset and unable to work or study, which seriously affects the patient's physical and mental health. Most convulsions stop after falling asleep.

5. In severe cases, the patient may suffer from rigidity, inability to open the eye on the same side, corner of mouth tilted to the same side, and inability to speak. The condition is often aggravated by fatigue, mental stress, and voluntary movement, but the patient cannot imitate or control the attack by himself.

6. Bilateral facial muscle spasm is very rare. If there is a seizure, the onset is usually one side at a time, with the convulsion on one side stopping before the convulsion on the other side. Moreover, the convulsion on one side is lighter than that on the other side. There have been no reports of convulsions on both sides at the same time. A small number of patients experience mild facial pain during convulsions, and some cases may be accompanied by ipsilateral headache and tinnitus.
