There are many ways to preserve the bladder, and most of them are based on comprehensive treatments of chemotherapy, radiotherapy and transurethral resection of bladder tumors. In the past, researchers have tried radiotherapy or chemotherapy alone, but the results were not satisfactory. Comprehensive radiotherapy, chemotherapy and surgery can save bladder cancer patients from having their entire bladder removed. Although current research has achieved some results, the standard treatment for muscle-invasive bladder cancer is still radical cystectomy, that is, radical cystectomy. If the lesion cannot be completely removed, chemotherapy and radiotherapy can be considered after surgery. Only about 40% of bladder cancer patients who choose bladder preservation treatment are able to successfully save their bladders.

Drugs for bladder cancer:

Urothelial carcinoma of the bladder is relatively sensitive to chemotherapy. For early-stage non-muscle-invasive bladder cancer, intracavitary chemotherapy or immunotherapy can be used after transurethral surgery to reduce the postoperative recurrence rate and delay tumor progression. For limited-stage muscle-invasive bladder cancer, the use of chemotherapy before and after radical surgery can achieve the purpose of downstaging, improving surgical resection rate, and prolonging survival. In addition, in the comprehensive treatment of bladder preservation, systemic chemotherapy can not only kill micrometastases, but also increase the sensitivity of radiotherapy. For advanced metastatic bladder cancer, systemic chemotherapy is the only treatment method that can prolong patient survival. Therefore, chemotherapy for bladder cancer has an indispensable position in the treatment of patients with bladder cancer of different stages and grades.

Surgery for bladder cancer:

1. Transurethral resection of bladder tumor

Transurethral bladder cancer tumor resection is both an important diagnostic method and a major treatment for bladder cancer. The tumor must be completely removed until the normal bladder wall muscle layer is exposed, and a base tissue biopsy is recommended. For patients with local recurrence, transurethral bladder tumor resection is recommended again.

2. Transurethral laser surgery

The efficacy is similar to that of transurethral resection of bladder tumors, but pathological diagnosis cannot be obtained and preoperative biopsy is required. In addition, it is not helpful for staging and is generally suitable for papillary low-grade urothelial carcinoma.

Photodynamic therapy for bladder cancer:

Photodynamic therapy can be considered for bladder carcinoma in situ, to control bladder cancer tumor bleeding and multiple tumor recurrences, and for those who cannot tolerate surgery.
