1. Androgen receptor blockers Androgen receptor blockers can compete with endogenous androgens and dihydrotestosterone in target cells to bind to receptors, inhibiting androgens from entering the cell nucleus, thereby blocking their biological effects. Androgen receptor blockers have the advantages of being easy to use and having few adverse reactions, so they are widely used in clinical practice. Androgen receptor blockers can be divided into steroidal and non-steroidal drugs. The drugs include steroidal drugs such as cyproterone acetate and megestrol acetate, and non-steroidal drugs such as flutamide and nilutamide.
Bicalutamide (Casodex) is a selective anti-androgen drug that is currently widely used. Its efficacy is 5 times that of flutamide, its effect on the central nervous system is relatively weak, its toxicity is low, and its adverse reactions are small. It is well tolerated by patients and does not affect sexual desire and sexual function.
2. LHRH analogs
LHRH (luteinizing hormone-releasing hormone) analogs can inhibit testosterone production by inhibiting the release of luteinizing hormone from the pituitary gland that acts on interstitial cells in the testis, thereby reducing the testosterone concentration in the blood to the castration level. Clinically used ones include leuprorelin acetate (enantone), buserelin, goserelin acetate, etc.
